The autonomy of selecting a home or community
Initial Consultation:
Discuss clinical, financial and geographic needs and preferences
Meet senior to get a feel for care needs
Establish the criteria for selecting a community or home
Relocation education regarding community types and what best meets the needs and preferences
Provide resources such as movers, realtors, pharmacy, attorney’s, pre-planned memorial etc.
Schedule a time to go visit the communities or homes
Tour homes and communities:
Pick up and drive (like a realtor) the senior and or family to 4-6 homes/communities
Personally visit the homes/communities to meet the providers and staff
Help guide the questions and advocate for the care and needs of the senior
Narrow down the options based on criteria and preferences
Make the selection:
Answer questions or concerns about each home/community
Revisit the final selection for a meal and paper work
Schedule the assessment
Client makes a deposit
Schedule the move
Move Day:
Available for family support, paperwork, words of encouragement, or that much needed hug.
Follow up:
A personal visit with a gift within a week of moving by our senior housing specialist
To ensure that you are adapting well and needs are being met
Address any potential concerns or requests with the provider and staff
Confirm our follow up communication with the resident and family
Offer our ongoing availability at any time to be an advocate for the senior
